Fall 2010 Player Registration -CLOSED-
A $25 Early Bird Discount applies to all registrations made online during the discount period. See programs for applicable dates. Discount does not apply to mail-in registrations.
Payments not received within 5 days after the close of registration, will be assessed a $25 Late Payment Fee. See Programs for applicable close dates.
Late Registrations are not accepted without prior approval of your child's Age Group Coordinator. A $25 Late Fee will apply to all approved late registrations.
For the U9 - U14 Traveling Program

The Mahtomedi School District is assessing all athletic associations that use
school facilities a $15.00 user fee per participant, per season. This means the
Mahtomedi Soccer Association will be assessed twice each year because we
have a fall and summer season.

Other Forms Required to Complete the Registration Process
Both of the forms listed above must be completed for each player registered and given to each player's respective coach prior to the first practice. Do Not Mail These Forms! U9 and older players will also be required to provide to their coach, a wallet size photo of themselves.

- Please follow the registration instructions below:
1. Completed Player Registration Form (online or by mail).
2. Completed & signed MYSA Emergency Information Consent Form (provide to coach, do not mail) .
3. Completed & signed MSA Code of Conduct Form (provide to coach, do not mail).
4. For U9-U18 players, a wallet sized photo of the player with name printed on back (provide to coach, do not mail).
5. Payment by credit card or check for online registration, check only for mail in registration.
6. Copy of the player's birth certificate (provide to coach, do not mail).
Waiver
Waiver to Mahtomedi Soccer Association (MSA): In consideration of your acceptance of my child as a participant in MSA. MSA, their Board of Directors, members, organizers, sponsors, coaches and persons transporting my child to and from activities, from and against any claim arising out of injury or harm to any child incidental to, connected with or arising out of MSA activities. All sports have intrinsic hazards. Participation in MSA implies acceptance of some risk of injury.
